PMCA Slide Show Program

This slide presentation consists of 75 slides, loaded in a carousel, with an outline and a printed script. You will need your own projector and screen.

To rent the PMCA slide show, make a payment of $20.00 per date after confirming your date(s) via a letter, email, or phone. The $20.00 cost covers first class postage, both ways (all labels are premade). We will also provide literature to hand out, free of charge. Please keep any extra handouts for future use.

Note: The PMCA maintains a page to advertise events. If you would like to display infromation about your program, please fill out this form as well.

There are two options for submitting a request:

1) Fill out this form, print it out, then mail it to us. We need your signature, agreeing to conditions stated below. Please sign, date, and return this form with payment. If you use this option, you must sign here:

2) Submit this form electronically. By doing so, you are aggreeing to our conditions stated below.

Conditions of Aggreement:

I would like to rent the PMCA slide show for the date(s) given above. After confirming the dates with the PMCA, I will make a payment of $20.00 for each date. [Payment can be check, money order, or credit card (Visa, MasterCard, or Discover card; please include expiration date), made out to PMCA.]

I will return the slides to PMCA in good condition immediately after the showing (next business day) and understand that the slides cannot be reproduced.

A prepaid postage meter strip for first class return postage will be enclosed with the slides. To return the slides, have your local post office date and cancel the meter strip, peel off the backing, and stick on the mailer. Mail to PMCA; 301 Peninsula Dr., Suite 6; Erie, PA 16505. Payment Options: Do not pay us until you recieve a confirmation. Slide shows have limited availability. We will ask you to pay if your request can be accomodated. Thank you.
